Description of Primary Responsibilities
Provides management to departmental staff.
Responsible for performance management of assigned supervisors, and those supervisors’ assigned staff, including recognition, performance evaluations, formal coaching and counseling, and making decisions or recommendations regarding necessary disciplinary actions.
Responsible for making recommendations regarding hiring or firing, and the advancement and promotion of assigned supervisors, and those supervisors’ assigned staff, or any other change status of assigned supervisors, and those supervisors’ assigned staff.
Demonstrates core leadership behaviors and team one approach.
Creates a culture of accountability and excellence.
Develops and manages an action plan across assigned employee base to support the strategic direction and obtainment of goals of the organization, effectively leading change when necessary.
Empowers staff through effective communication and talent building.
Delegates appropriate departmental duties and responsibilities to assigned team members while ensuring department performance.
Assists with the development of the departmental budget and monitors budget to ensure expenses do not exceed budget.
Ensures regulatory compliance for assigned departments, and compliance with all workflows, policies, and procedures.
Ensures department employees receive instruction/training that is in compliance with training plan, including on the job training to develop department employees and supervisor.
Works with supervisor to ensure necessary remediation is taken.
Recommends workflow, policy, and procedure changes based on observations from performance metrics, outcomes, and feedback from supervisor.
Ensures department maintains compliance with all employee related reporting and tracking.
Coordinates all center activities including care planning and implementation, preparation for interdisciplinary conferences and daily meetings, family conferences, and initial and period assessments.
Supervisory responsibility for registered nurses, rehabilitation therapists, clerical staff, social workers, transportation staff, recreational staff, registered dietitian, and center aides.
Facilitate and lead morning meetings of the interdisciplinary team (IDT); facilitate and lead regularly scheduled Intake and Assessment meetings to review participant cases on a regular basis.
Oversee the quality of care and safety of FHCN PACE participants, both at the FHCN PACE center and in the community.
Serves as a liaison between disciplines and between administration and direct staff.
Works closely with the medical care team as part of the IDT to develop and implement participants’ individualized care plans.
Coordinates 24- hour care delivery.
Coordinates with the marketing department to hold “friendly visits” for new candidates at the FHCN PACE center.
Serves as a representative of FHCN PACE to the community.
Coordinates activities related to new participants.
Manages the center’s budget; communicates regularly with FHCN PACE administration on center operations and needs.
Oversees overall documentation cohesiveness and accuracy in each participant’s medical record.
Other projects and duties as assigned.
Description of Primary Attributes Professional & Technical Knowledge: Bachelor’s degree in a health-related field required, Master’s degree preferred.
At least 3 years’ experience in a supervisory capacity within a healthcare setting.
Either have one year of experience working with a frail or elderly population or, in the absence of such experience, to receive appropriate training from the FHCN PACE on working with a frail or elderly population upon hiring.
Candidate must be comfortable working in a dynamic environment and comfortable working with both direct care staff and administrative staff.
Experience in successfully managing and completing projects is required.
Experience with senior care clinical programs similar to FHCN PACE preferred.
Must excel in organizational and supervisory skills, with an ability to effectively handle difficult situations.
Experience working with clinical charts and records.
Pass a criminal background which will verify that the candidate is: not excluded from participation in Medicare and Medicaid programs; has not been convicted of criminal offenses related to their involvement in Medicaid, Medicare, other health insurance or health care programs, or social services programs under title XX of the Act; and have not been convicted of physical, sexual, drug or alcohol abuse.
Technical Skills: Work independently and as an effective member of a team.
Document participant changes appropriately in the medical records and communicate participant changes to IDT in a timely manner.
Participate as a member of the IDT.
Attend staff meetings and take part in participant care planning.
Be medically cleared for communicable diseases and have all immunizations up to date before engaging in direct participant contact.
Lead and facilitate IDT decision making.
Trouble shoot efficiently to maintain quality operation of the FHCN PACE center.
Be organized within busy center and clinic environments.
Establish and maintain effective interpersonal relationships with all levels of staff, other departments, agencies, members, providers and the general public.
Communicate effectively, both verbally and in writing, with individuals from varying cultural backgrounds.
Prepare concise and clear reports.
Identify pertinent policy and operational issues; develop strategies, solutions and recommendations.
Multi-task in projects and their respective activities, timelines and issues.
Effectively utilize computer and software tools.
Only act within the scope of his/her authority to practice.
Meet a standardized set of competencies for the specific position description established by FHCN PACE and approved by regulatory agencies before working independently.
License & Certifications: None.
Be medically cleared for communicable diseases and have all immunizations up-to-date before engaging in direct participant contact.
Currently certified in cardiopulmonary resuscitation (CPR), if CPR is not current, will be granted 30 days from date of hire to schedule for next available CPR certification class provided by FHCN.
Communications Skills: Job duties require the employee to effectively communicate, verbally and in writing, their opinions and extrapolations of information they collect and synthesize/analyze.
Responsible for the resolution of conflicts that may arise because of disagreements between employees, between employees and customers/clients, or with the public, other legal entities or governmental authorities.
Compiles, analyzes and prepares information in an effective written form, including correspondence, reports, articles or other documentation.
Effectively conveys technical information to non-technical audiences.
