Benefits Eligible Full-Time Benefit Eligible Work Shift Variable (United States of America) Department 7643 SRCC Rad Onc-Visalia Manages the care of identified patients who are high risk for lung cancer.
Provides program oversight that may include additional cancer diseases.
Oversees the daily operations of the Lung Screening program and collaborates care by working with referring physicians for incidentally found lung nodules.
Functions as a clinical advocate and educator for patients as well as assisting in building and maintaining community relationships to provide expedient and reliable access to quality care.
QUALIFICATIONS License/Certification Required: CA RN license BLS Oncology Certified Nurse (OCN) within 18 months of hire/transfer Education Required: Bachelor's degree in Nursing, Healthcare, or related field Experience: Required: Minimum of two years of experience in Oncology working with cancer patients and their families Preferred: RN navigator experience JOB RESPONSIBILITIES Essential Maintains navigation software for tracking patient’s lung screenings and incidental findings in the lungs.
Is responsible for overseeing the gathering of required information for the screening program.
Functions as a primary contact for patients assuring efficient access to care, education to patients, families and the community.
Coordinates patient information with referral services and works with physician offices and clinics to manage follow up testing.
Communicates closely with physicians in coordinating and facilitating diagnostic and therapeutic procedures as needed.
Works in conjunction with SRCC Oncology team to serve as a consistent contact for patient/family throughout the process of diagnosis and treatment.
Assesses and facilitates coordination of multidisciplinary care across the care continuum.
Identifies patient and/or provider needs and ensures delivery of quality services to patients.
Maintains navigation services documentation according to program standards.
Assists cancer patients across the continuum of care in accessing the hospital’s cancer care delivery system.
Effectively communicates with patients, caregivers and members of the multidisciplinary team.
Assesses patient’s needs by identifying patient and family barriers.
Bridges gaps in care, by assessing/troubleshooting care transitions and barriers that may exist.
Provides access and information to resources available within the hospital system and the community.
Maintains advanced knowledge of the assigned cancer disease site, oncology nursing and oncology navigation by attending continuing education programs.
Practices according to professional and legal standards.
Meets ONN annual competencies and job performance standards Attends multidisciplinary tumor board and chart rounds at cancer center.
Demonstrates the knowledge and skills necessary to provide care and services appropriate to the population served on the assigned unit or work area.
Knowledgeable of growth and development for all patient/family cultural, linguistic, spiritual, gender, and age specific needs.
Able to effectively communicate and care for patient and family as reflected in the Plan for Provision of Care.
Performs other duties as assigned Pay Range $84,198.
