Putnam Avenue Porterville, CA 93257 559-781-3700 *JOB DESCRIPTION * *Job Title: Licensed Clinical Social Worker * *Job Summary:* Under the direct supervision of the Medical Director, The Licensed Clinical Social Worker is responsible for diagnosing, treating, monitoring, and counseling patients with mental, behavioral, and/or emotional disorders while effectively collaborating and communicating with the Medical Director, Providers, and other healthcare professionals.
*Job Duties: * This position is responsible for the following: * Provide great care to SFMC’s patients with behavioral, mental, and/or emotional health problems.
* Collaborate and communicate effectively with Primary Care Providers and Healthcare personnel involved with patient care and treatment.
* Conduct mental health assessments to patients.
* Assist with development and improvement of programs as well as departmental policies, procedures and protocols.
* Demonstrate excellent customer service with patients.
* Maintain communication with patients by returning patient calls and follow ups for referrals.
* Effectively write clinical reports regarding patient diagnoses and prognoses.
* Able to multitask and manage assigned projects and tasks as directed.
* All other duties and responsibilities as assigned.
*JOB REQUIREMENTS: * * Must be licensed in the State of California as a Licensed Social Worker.
* Ability to work under pressure.
* Ability and willingness to treat all patients and co-workers with the utmost kindness and consideration in the most trying situations.
* Ability to handle multi-functions.

* Ability to relate to public regardless of ethnic, religious and economic status.
* Must possess or obtain a Valid California Driver’s License.

*QUALIFICATIONS, EDUCATION, AND EXPERIENCE: * * Must have a Master’s Degree in Social Work from an accredited school.
* Experience in behavioral healthcare, including substance use disorders, and psychotherapy practice preferred.
* Knowledge and experience in in psychotherapy treatment including, but not limited to, cognitive and behavioral, postmodern/post-structural, social- constructionism, and narrative therapy is a plus.
* Ability to demonstrate thorough knowledge and expertise in reporting laws as well as all legalities of care relevant to the specific population serviced.
* Ability to demonstrate excellent independent judgment, problem-solving, analytical and collaborative skills in order to organize and prioritize multiple tasks, providing patient centered leadership and direction.
* Must have the emotional intelligence that may include, but not limited to, patience, sensitivity and the compassion to work with patients that may be stressed, anxious, and difficult.
* Ability to work as a team.
* Ability to demonstrate knowledge and willingness to engage in learning about various spiritual, ethnic, and culturally diverse backgrounds.
* Bilingual in English/Spanish is preferred.
* Must have strong computer skills.
* Must demonstrate detailed, effective writing/documentation and verbal skills.
*Reports to: Medical Director* *Supervises: NONE * *Classification: *Full Time Position, Non-Exempt *SALARY: $65,000 - $85,000 per year, DOE* Job Type: Full-time Pay: $75,000.
