SUMMARY
The purpose of this position is to engage potential clients, provide outreach in low-income communities as well as determine eligibility and enrollment for Senior Services programs.
ESSENTIAL DUTIES AND RESPONSIBILITIES include the following.
Other duties may be assigned.
Collects, organizes, and analyzes information about individuals through assessments and records to evaluate the interests and needs for service planning.
Maintains client notes, intake, and files; documents communication and referrals to other partners and provides progress notes regarding client outcome.
Interprets, adheres to, and implements program directives for program elements, design, and interpretation of Kings/Tulare Area Agency on Aging contract.
Assists in developing plans and strategies to meet contractual goals and obligations including assistance with enrolling clients in programs and evaluating client progress to determine services.
Develops and evaluates courses and training materials to educate clients and the community about elder abuse prevention, community resources, etc.
Complies with all HIPAA regulations and mandated reporting procedures.
Serves as liaison between program partners to provide education and customized activities and events which include but are not limited to: nutrition education, health promotion, disease management, etc.
Develops activities to assist clients in socialization while encouraging participation at the Senior Centers.
Recruits, trains, and schedules volunteers to assist with congregate meals while providing guidance and direction to volunteers as needed.
Provides enrollment and termination information to Senior Services intake staff in a timely manner.
Obtains and maintains "ServSafe Certification" to aid in food safety knowledge.
Performs other duties as required.

EDUCATION and/or EXPERIENCE Preferred- Possession of Bachelor's Degree (B.

Preferred- Two years of recreation or event leadership experience.
Preferred- Two years of experience in providing services to low-income persons.
LANGUAGE SKILLS Ability to speak in both English & Spanish is desirable.
Ability to read and interpret documents.
Ability to write routine reports and correspondence.
Ability to speak effectively with groups of customers or employees of the organization.
MATHEMATICAL SKILLS Ability to calculate figures for recording and tracking purposes.
Ability to apply basic recordkeeping principles.
REASONING ABILITY Ability to apply common sense understanding to carry out instructions furnished in written and oral form.
Ability to deal with problems involving several concrete variables in standardized situations.
CERTIFICATES, LICENSES, REGISTRATIONS California Driver's License and proof of auto insurance upon hire.
Possess or have the ability to acquire "ServSafe Certification" within 2 months of hire.

Gerontology knowledge preferred; Bilingual preferred.
