Benefits Eligible Full-Time Benefit Eligible Work Shift Variable (United States of America) Department 8516 Care Coordination Grant A Community Health Worker (CHW) is a frontline public health worker who is a trusted member of and/or has an unusually close understanding of the community served.
This trusting relationship enables the CHW to serve as a liaison/link/intermediary between health/social services and the community to facilitate access to services and improve the quality and cultural competence of service delivery.
A CHW also builds individual and community capacity by increasing health knowledge and self-sufficiency through a range of activities such as outreach, community education, informal counseling, social support and advocacy.

Ensures adequate supplies of incentive items are maintained to supply all planned outreach events.
SNAP ED NUTRITION EDUCATION PROGRAM:Assists the Project Coordinator with nutrition education program planning and development.
Provides nutrition education to the Nutrition Education Obesity Prevention (NEOP) eligible population through group presentations, model a lesson, displays, health fairs and other events.
Provides a Train the Trainer for early childcare teachers and staff on a state approved nutrition education curriculum.
Provides ongoing technical assistance and support to teachers and staff on approved nutrition education curriculum.
Conducts nutrition education, physical activity promotion interventions in the community setting by planning and implementing events such as the Dinuba Farmers’ Market and the Latino Health Awareness Day.
Works with Community Based organizations to increase the reach of nutrition education interventions to NEOP eligibles.
Tracks nutrition education lessons and activities conducted by trained teachers and maintains state developed database up to date.
CHRONIC DISEASE MANAGEMENT PROGRAM:Become certified as a Master Trainer in selected chronic disease management programs.
Maintains eligibility requirements to remain an active certified Master Trainer in selected chronic disease management programs.
Conducts 1-2 chronic disease management program Train the Trainer Facilitator workshops per year.
Maintains a list of individuals who have completed Train the Trainer Facilitator workshops and ensure that they are meeting all necessary requirements to remain active facilitators.
Works with Coordinator, Kaweah Health Marketing Department, active Facilitators, community based organizations and partners to plan and promote chronic disease management workshops being offered in the community.
Assists Coordinator with scheduling active facilitators to conduct chronic disease management workshops.
Facilitates and/or co-facilitates chronic disease management workshops.
Observes workshop facilitators to ensure that curriculum fidelity implementation is being met and collect all required program forms at the conclusion of the workshop.
Monitors program supplies and work with Coordinator to place orders as needed.
Works with Coordinator and data analyst to ensure that program statistics are entered into data collection system.
Works with Coordinator to complete and update program reports.
Utilizing a company vehicle, assists with transporting Chronic Disease Management Center patients to and from their appointments on an as needed basis.
Additional Advises the Community Outreach Coordinator and Director on the expansion of program services.
May identify and coordinate continuing education training for self and program promotores.
Participates in the screening and selection of health education materials including audiovisual and print materials.
Responsible for all education supplies, equipment, and materials.
Maintains inventory and check out system.
Provides any additional in-services/ trainings that may benefit staff in understanding/serving the Latino community.
Promotes program services and works with agencies to develop effective referral mechanisms and opportunities for community education.
